School Physicals Information/Form
CHANGES IN HEALTH OFFICE MANDATES
PHYSICAL AND DENTAL EXAMS
Starting this September 2018 school year, New York State Education Department has changed the mandated years students need to have a physical and dental exam. Please note the changes. The following grades will need a physical exam done:
- UPK or Kindergarten
- Grades 1, 3, 5, 7, 9 and 11
- Any student new to the West Seneca School District
We strongly encourage you to have the physical exam performed by your child's doctor, but if that exam is not received by the school nurse, one will be done by the school physician. You will receive one written notice regarding this before the exam is done. If your school has a monthly building calendar, it will be noted in the calendar the week the physician will be in. If you have problems having the physical done by a private physician, contact your school nurse.
New York State will be mandating that medical examiners use one specific form to use for school physicals. Your School Nurse will keep you updated on this information.
Students participating in interscholastic sports need a yearly physical.
