Physical Appointment Scheduler

    PLEASE do NOT make multiple appointments.  
    You can change or cancel your appointment by clicking on the appropriate button at the bottom of your confirmation email.
    If you make an appointment PLEASE arrive at YOUR SCHEDULED TIME.

    West Seneca CSD is accepting appointments for athletic physicals (grades 7-12) for the 2024-2025 school year.

    NOTE - If your son/daughter had a physical within the past year (by PMD or school provider) - you may only need to complete an update form.  (i.e physical done 8/15/2023 is valid until 8/31/2024 with a completed update form done through the registration process).

    NOTE: you can find your child's most recent physical date on the parent portal under the athletic registration link.

    To obtain a physical at the school, please follow these directions.

    STEP 1.   Download this Form (For on-premise physical ONLY)

    Complete the top section of this form.  Print out completed form.  PARENT SIGNATURE REQUIRED.
          This form MUST be brought with the athlete to the physical appointment.


    STEP 2.    Schedule an Appointment
    Note:  you will receive a confirmation email and then a reminder prior to your scheduled appointment.


    The information provided will be accessed only by the Athletic Department for the purpose of scheduling physicals.
