Incoming 6th Grade Students

All 6th graders are mandated by New York State to have a TDaP vaccine once they turn 11 years old. Please contact your child’s physician for an appointment for this mandated vaccine. Please forward a copy of your child’s updated immunization record to the health office once the needed vaccine has been given. Thank you.

Incoming 7th Grade Students

New York State mandates that all students entering 7th grade in September have the Meningococcal vaccine at the start of school in September. Please contact your child’s physician for an appointment for this mandated vaccine. Please forward a copy of your child’s updated immunization record to the health office once the needed vaccine has been given. Also note that all 7th grade students are mandated to have a physical exam when in 7th grade. Please contact your child’s physician for a physical exam and forward it to the health office.
