    This form is for Current Students that will be moving to another address
    within the West Seneca Seneca Central School District 

    Please return the completed form to the building your child will be attending (see below)

    Completed Change of Address Forms will be accepted in the following manner:

    1) Print and return - by appointment only - 
            a)  To request an appointment, phone your school or 
                  click the "Email Contact" link to the right of the school your 
                  child will be attending.
    2) Fax to building your child will attend
    3) USPS Mail to building your child will attend.
